Before you can begin pre-tax payroll deductions for eligible Section 125 expenses, you need to have Cafeteria Plan Documents in place. In order to have PBS, Inc. prepare the documents, you will need to complete a Document Preparation Sheet. Click on Documents. You will find preparation sheets for all the plan documents. You will also need to complete a service agreement. A Full Agreement is required if you are going to be utilizing our monthly administration services. A POP Agreement is required if you only want us to set up a Premium Only Plan or self-administered HRA Plan for you.

Remember, an IRS form 5500 and any related schedules may need to be completed for your Section 125 Cafeteria Plan each year.
